Working under the direction of the patient's doctor, the nurse is responsible for completion of the Initial Assessment Visit, implementation of the Plan of Care, and coordination of the services the patient receives from admission/referral to discharge.
CERTIFIED HOME HEALTH AIDE
Certified Home Health Aides (CHHA) have had special taining in Home Health care giving. CHHA's help patients with personal care services such as grooming, bathing, meal preparation, and light housekeeping.
Phystical Therapists (PT) provide evaluation, treatment, and instruction to increase range of motion, strength, and endurance,. Physical Therapists may help patients with gait training, use of wheel chairs, use of walkers, artificial limbs, and pain control.
Occupational Therapists (OT) help each patient learn to better perform activities of daily living. These activities include eating, grooming, dressing, and toileting
MEDICAL SOCIAL WORKER
Medical Social Workers (MSW) provide social, environmental and emotional counseling for the patient and families.
To facilitate the delivery of professional and ancilliary Home Health Care Services needed in the patient's home setting
To promote good health and independent functioning to preserve dignity and quality of life for aging and medically challenged ones.
To facilitate early hospital discharge while providing peace of mind to the patient and family that QUALITY HEALTHCARE is our COMMITMENT to you.
Direct care is provided by the primary caregiver, if available, and augmented by the home health team. Care is provided wherever the Patient calls home.
The home health team consists of:

· Certified Home Health Aides
